ACCEPTING MY CHILD(REN) ENTRY, I HEREBY, FOR MYSELF, MY CHILD(REN), MY HEIRS, EXECUTORS, AND ADMINISTRATORS, WAIVER AND RELEASE ANY AND ALL RIGHTS AND CLAIMS FOR DAMAGES I OR MY CHILD(REN) MAY HAVE AGAINST HELP THE CHILDREN FOUNDATION, INC. AND ITS REPRESENTATIVES, SUCCESSORS, AND ASSIGNS FOR ANY AND ALL INJURIES, DISEASES, VIRUSES OR UNFORESEEN DEATH SUFFERED BY MYSELF AND MY CHILD(REN) AT THE PROGRAM LISTED ABOVE.
